{"id":26349,"date":"2023-09-07T18:30:00","date_gmt":"2023-09-07T18:30:00","guid":{"rendered":"https:\/\/adn.monetizemail.com\/?p=26349"},"modified":"2023-09-08T06:41:00","modified_gmt":"2023-09-08T06:41:00","slug":"two-step-solution-to-improving-veteran-colon-health","status":"publish","type":"post","link":"https:\/\/adn.monetizemail.com\/?p=26349","title":{"rendered":"Two-step solution to improving Veteran colon health"},"content":{"rendered":"<p>Antibiotic overuse can lead to serious infections from <a href=\"https:\/\/www.veteranshealthlibrary.va.gov\/DiseasesConditions\/InfectiousDiseases\/3,89209\">Clostridium difficile<\/a> (C. diff), a bacterium that affects the colon. Veteran patients in hospitals and long-term care facilities are more vulnerable to C. diff infections than those not receiving care in hospitals or long-term care facilities.<\/p>\n<p>From 2019-2020, C. diff rates rose sharply in the <a href=\"https:\/\/www.va.gov\/columbia-south-carolina-health-care\/\">Columbia VA<\/a> Community Living Center (CLC). This led to increased lengths of stay and heightened Veteran patient safety risk.<\/p>\n<p>In 2021, a process improvement team worked on the <a href=\"https:\/\/marketplace.va.gov\/innovations\/reducing-hospital-acquired-c-dff-rates-for-inpatient-and-clc-veterans\">two-step solution to fixing the C. diff problem<\/a> at Columbia VA. The process improvement innovation promotes appropriate use of antibiotics, embeds best practices at the point of care and incorporates staff education on two-step testing.\u00a0<\/p>\n<h2 class=\"wp-block-heading\">Improving on traditional methods with two-step solution<\/h2>\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-large\"><a href=\"https:\/\/adn.monetizemail.com\/wp-content\/uploads\/2023\/09\/Team-Photo_r1-2.jpg\"><\/a><figcaption class=\"wp-element-caption\">C. Diff team left to right: Evanne Leblanc-Wyckoff, Lucy Austin, and Paula Guild<\/figcaption><\/figure>\n<\/div>\n<p>C. diff testing is typically done through polymerase chain reaction (PCR) testing, a rapid diagnostic test for infectious diseases. However, this test can result in false positives. The team recommended a key second step to improve Veteran care, which ultimately reduces C. diff rates in hospitals. This second step was adding a toxin testing for PCR positive C. diff samples.<\/p>\n<p>With this second step, clinicians can identify a toxigenic or toxin producing C. diff infection and appropriately prescribe antibiotics, when needed. <\/p>\n<p>\u201cIf we have somebody who is identified as not having toxigenic C. diff, then you may have a lower length of stay in the hospital. That can decrease your risk to other types of hospital acquired infections,\u201d said the infection prevention team. They noted how the practice complies with the current recommended testing methodologies for patients.<\/p>\n<h2 class=\"wp-block-heading\">Putting it to the test<\/h2>\n<p>The initial pilot results were impressive. C. diff rates were reduced by 51% in acute care and 100% in the CLC after less than a year of implementing the program. The reduction in C. diff rates resulted in estimated facility savings of $375,000.<\/p>\n<p>The team entered their practice into Diffusion of Excellence\u2019s 2022 <a href=\"https:\/\/marketplace.va.gov\/competitions\/shark-tank\">VHA Shark Tank Competition<\/a> and was selected as a winner to replicate at the <a href=\"https:\/\/www.va.gov\/central-arkansas-health-care\/\">Central Arkansas VA<\/a>.<\/p>\n<h2 class=\"wp-block-heading\">Spreading positive results<\/h2>\n<p>With Diffusion\u2019s support, the team pitched to national VHA program office stakeholders and made significant progress to replicate at Central Arkansas VA. \u201cIt\u2019s been a phenomenal experience,\u201d said the infection prevention team. The success of this project through this national outlet has helped bring a focus to our facility about what process improvement projects can do and how much of an impact you can make.\u201d<\/p>\n<p>The team is excited to engage other sites in the practice, and plans to enter other innovations in future rounds of VHA Shark Tank Competition.
